Insurance Requirements

Insurance companies vary their requirements from carrier to carrier and sometimes from case to case, depending on a variety of factors.  Exams typically require blood and/or urine.  Additional requirements may or may not be a part of your required process.  Make sure your order reflects the needs of the company with whom you are working.

Additional requirements may include electrocardiogram (EKG), often depending on policy amount and/or age.  If an actual physician examination is required, scheduling arrangements may be made.
